| Land Use Review forms provide the City Planning Commission
(CPC), Department of City Planning (DCP), community boards,
borough presidents, the City Council and other interested
parties with information needed to evaluate applications
for many land use actions subject to city review.
Most
land use actions are subject to the Uniform Land Use
Review Procedure (ULURP), a public review process mandated
by Sections 197-c and 197-d of the City Charter. Zoning
text amendments follow a similar process but are not
subject to ULURP. Other actions require approval by
the City Planning Commission or its chair but are not
subject to ULURP. Such actions include zoning authorizations,
certifications, review of Charter Section 197-a plans,
UDAAP area designations and project approvals, business
improvement districts, and modifications or follow-ups
to previous land use actions.
The
online version of the forms contains instructions and
forms used for submission of applications to the NYC
Department of City Planning for approval of land use
actions. It includes a complete set of forms and instructions
describing how to complete the forms and the required
attachments for each of the following actions:

The forms and instructions were modified in May 2005 (identified by an 0505 at the lower left hand corner of each page). Please note that these forms must now be used for new submissions. Applications using the old forms will not be accepted after September 30, 2005.

The Word and WordPerfect forms and instructions are downloaded
as zipped files. Select the preferred download link,
below. You will be prompted by your browser to save
the file to disk and to select a destination directory
on your local system. The forms can be extracted from
the zip file using WinZip, PKZIP or a similar utility.
Included are nine files for each format: eight forms and
the initial instructions, readme. Please look
at the readme file first for further instructions on
using these forms.

Instructions
and forms may be viewed on the screen or printed. The
forms can be downloaded to your local computer, however
they cannot be filled out electronically.
Print
the instructions and forms with legal size paper (8
1/2" x 14").
The
downloaded copy will be similar to the published ULURP
application package available at the Department's Map
and Book Store at 22 Reade Street, for $3.00. Once printed
from the PDF file, forms may be completed on a typewriter
and submitted to the Department with the required attachments
and in the appropriate number of copies.

NOTICE TO APPLICANTS:
Pursuant to Local Law 34 of 2007, as amended, the City is required for the purposes of the Campaign Finance Law to establish a computerized database containing the names of any “person” that has “business dealings with the city”. This term is defined in the law to include the processing of applications under NYC Charter §197-c (ULURP) and §201 ( Zoning Text Amendment).
In order for the City to obtain information necessary for the database, the Department of City Planning/City Planning Commission has adopted an amendment to Chapter 2 of Title 62 of the Rules of the City of New York, which requires that a ‘Doing Business Data Form’ be submitted with all land use applications covered by Local Law 34.
Subject to limited exceptions set forth in Local Law 34, beginning on April 14, 2008, no application for the processing of a land use action under ULURP or for a Zoning Text Amendment will be accepted by the Department of City Planning, unless the ‘Doing Business Data Form’ has been filled out in its entirety. Likewise, on or after April 14, 2008, applications covered by Local Law 34 will only be certified or referred for public review if the ‘Doing Business Data Form’ has been completed. Note that the ‘Data Business Data Form’ must be completed for any applications covered by Local Law 34 which were filed prior to April 14 and have not been certified or referred to date.

Completed forms should be included in all new application packages filed at Central Intake, Department of City Planning, 22 Reade Street.
