Street, Highway & Sidewalk Construction Complaint Form

The Highway Inspection and Quality Assurance (HIQA) unit regulates and enforces the laws and rules that govern the way construction work is performed on the City's sidewalks, roadways, and highways. To file a complaint about a street or roadway condition related to street excavations, placement and use of construction material, compliance with construction permits, sidewalk and curb defects or encroachments, and similar issues fill out and submit this form. To report a pothole or other street defect, call 311. Operators are available at all times to take your information and refer it to our pothole repair crews. You can also use our online form. Other forms on this Web site can be used to report street lights, traffic signals, traffic signs, potholes or other street defects in need of repairs. Or you can contact the Department to report other transportation-related problems. Failure to remove snow and ice from a sidewalk should be reported to the Department of Sanitation. Video and Picture Upload
You can upload up to three (3) files.
You can upload a total of three files, but the total size of all files uploaded can not exceed 3 MB.

  • Videos must be in .mov, .mpg, .mp4, .avi, .wmv, .3gp, .3g2, .3gpp2, or .rm format.
  • Pictures must be in .jpg, .png, .gif, .bmp, or .tif format.
  • Audio files must be in .mp3, .mp2, .wav, .wma, .aac, or .ra format.


Before uploading, make sure that all the videos and pictures you are sending are appropriate and relevant to the service request to which they are to be attached.

By uploading images to 311 you agree that the City may use the images for whatever purposes it sees fit. The City cannot guarantee that images will be reviewed or relied upon by its investigators. However, there are several things you can do to improve the usefulness of your image submission:

  • Submit images with high resolution
  • Make sure images are not blurry, and that the images clearly indicate the thing complained about
  • Make sure that you upload images that correspond to your complaint. The City cannot return or delete images that you did not intend to submit, even if those images are personal in nature. Individuals who submit offensive or harassing images not related to City business may be liable for violations of law.

To submit multiple complaints without having to fill out the entire form again, please follow these steps: After submitting the form for the first time, hit the back button and the information that you submitted will still be on the page. You can then make any necessary changes and hit the submit button again. Please note that if you reload the form or if you turn off your computer and return to the form at a later time, the old information will not reappear and you must fill out the form again.
