Chronic diseases are a serious problem across the United States. In New York City, they disproportionately impact many of the communities served by HHC. The management of diseases such as asthma, diabetes and heart failure is a major challenge for healthcare providers. HHC has stepped forward with a combination of patient engagement strategies and best practices to minimize complications, reduce hospitalizations and prolong lives. Collaborative chronic disease teams from HHC hospitals meet regularly and share a range of best practices that, when consistently applied, can be successful in improving patient outcomes.
HHC has utilized advanced clinical
information systems in the management of chronic diseases to more effectively inform clinicians and engage patients. A good example is our success with asthma action plans which are embedded in the electronic patient record of our asthma patients. Likewise, our electronic diabetic registry allows doctors to monitor the progress of their diabetic patients, and provide more targeted, evidence-based treatment.
By measuring several key indicators, we're able to track how successfully we are helping our patients manage chronic disease and improve their long-term health.
