|  Application Package When you are ready to lease a store property from NYCHA, you must complete the Store Application Package (Parts I, II and III) and the VENDEX Questionnaire (Vendor and Principle Questionnaire). The Space Application Form with all the required documentation can be completed on line but must be printed, signed and submitted per the instructions on page 1 of the form. For ease of use, Part II of the Commercial Space Application Form includes an auto-sum/tabulation function. The VENDEX Questionnaire is a form issued and reviewed by the Mayor’s Office of Contract Services (MOCS), and has to be completed and submitted per the instructions on that form.
