CIGNA HealthCare for Seniors


To enroll in this plan, you must obtain a special enrollment application directly from the health plan.

CIGNA HealthCare for Seniors is available to retirees with Medicare Parts A and B who reside in Phoenix, Arizona (Maricopa and the City of Apache Junction only).

With the CIGNA HealthCare for Seniors plan, you are subject to a $5 copay for PCP visits, $15 copay for specialist visits at CIGNA HealthCare Centers; copays vary for visits to other providers contracted by CIGNA. Plus you will find extras, like annual physicals and worldwide emergency care.

CIGNA HealthCare for Seniors virtually eliminates paperwork. Each time you go for a visit, you simply show your CIGNA ID card when using a plan provider.

Prescription Drugs         
Retail:  $10/$30/$30/$30 – 30 day supply
Mail:  $25/$85/$85/$85 – 90 day supply 
Copay coverage through  “DONUT HOLE” up to $4,050; after member reaches $4,050 member then pays $2.25/$5.60 or 5% coinsurance (whichever is greater).
